ABOUT US

Our priority is to bring high-quality and professional experience to Southeast Tennessee. We made this decision after years of witnessing Polk County patient’s traveling long distances to seek physical therapy services. Polk Physical Therapy was started in 2022  to address that need and to bring world-class services to residents of Polk County and the surrounding area.

We believe in not only operating our clinic locally but actively contributing to our community. We want to be a source of pride for the residents of our region and to be in network with every possible insurance plan as a commitment to care for our community.

WHAT TO KNOW

Getting Started

Appointments with our clinic are usually initiated by an order from a physician. Although your physician will send the order, you get to choose where you want to receive services. You can request Polk Physical Therapy, and your order will be faxed to our clinic, or simply call us with the request to set up an initial evaluation. Our front desk will contact your physician as needed.

Your First Session

In your first visit, we’ll perform a comprehensive evaluation of your problem, educate you on your diagnosis or surgery, and work with you to develop an appropriate plan of care. For each session, we ask that you dress in a manner that allows for both mobility and access to the joint that you are seeking treatment for.

Before Your Visit

Once your appointment is scheduled, we’ll send you a link via email where you can fill out all your intake information. We find that new patients like having this option – everything you want us to know will automatically be placed in your chart prior to your first visit. If you don’t have email or access to a computer, you can always fill out your paperwork upon arrival.

Our Hope for You

As our patient, we want you to leave your initial session excited that you both understand your diagnosis and there is a plan to get out of pain. We understand that  it’ss your decision to go where you feel most comfortable, and we will always work to make sure that Polk Physical Therapy is not just a convenience, but your best choice for rehabilitation.
